Outreach Support in your own home (Essex Cares)

 

Facilities

The aim of the Outreach service is to support you in retaining or learning new skills in order to remain living independently within your own home. We can also give you the confidence to start enjoying yourself again with the reassurance that someone will always be there to support you.

Outreach will provide you with a tailor made 1 to 1 support programme within your own home based upon your individual goals and aspirations. You can choose the types of support that you want and agree with your support worker how this is going to be achieved and delivered.

The Service can offer a variety of support and activities including help in managing home affairs, carrying out daily routine activities within the home or enjoying favourite pastimes and hobbies offering support to keep your mind and body active through activities of your choice.

Cost

Prices for sessions vary depending on the level of support required for each individual.

Provision of one to one support in the community relating to independent living including support with housing issues; attending appointments; shopping; emotional support; interests and activities

We offer the same services to those who need more support due to factors such as challenging behaviour or severe mental health. 2 members of staff will attend each service users home to carry out the session.
