<p>Home from Hospital is a friendly and confidential service providing one-to-one short-term practical support by a trained volunteer which enables patients to return home with confidence when no other support is available from friends or family. Volunteers help with shopping, collecting prescriptions, looking after pets or just being there when people begin to do things for themselves.</p> <p>Need a lift to this service?
